Claim Filing: Our medical team will sign and file your C-4 form with your employer’s insurer or Third-Party Administrator (TPA) to officially start your claim.

Initially, for emergency care, you may go to any facility. However, for your claim to be valid in Nevada, you must transition to a provider on your employer’s Managed Care Organization (MCO) or Preferred Provider Organization (PPO) list. The C-4 form is the "Employee's Claim for Compensation." It is the most critical document in your case. It must be filled out by a physician at the time of your first treatment. Without a signed C-4, your workers' compensation claim cannot be officially opened.
If your claim is accepted, the workers' compensation insurance carrier pays 100% of authorized medical expenses. You should not have a co-pay or out-of-pocket cost for authorized work injury visits.
